As you can see, the Quiz-o-matic '76 provides a series of questions, gives the test taker their results, then tracks those results in a database and presents them in a consise format to you, the webmaster. Pretty straight forward, right?

Quiz-o-matic Quiz Helper

Check out this uber-cool tool that Dustin Jackson made to automate the HTML making process for the Quiz-o-matic. It can support up to 100 questions, with options for true/false, short answer, and up to 7 multiple choice answers:

This is one of the best scripts here. You just have to appreciate just how good this little questionnaire is. Presents everything in a nice format and useable. Easy to use.

I had to convert some quizzes, which I originally wrote in Javascript, to PHP in order to make results tracking easier. I looked up some half a dozen PHP scripts and this one is by far the best. ...the script is clearly written, and, though I never used PHP before, I was able to understand how it works and make necessary modifications.

Updates/Fixes

04/22/05 - Fixed a problem with the name not being inserted in the database.
02/07/05 - Took out redundant use of $score variable.
12/23/04 - Fixed MySql version issue.
12/04/04 - Fixed bug on some systems where graph didn't show up.
11/29/04 - Fixed bug on some systems where all but last question was marked wrong.
10/02/04 - Updated code to run properly on PHP 4.
08/11/04 - Fixed Create_Table function to allow more than 127 entries in database.
